New Horizon Chiropractic & Wellness is a private-pay practice and a non-Medicare provider. Appointments with Dr. Kosciusko are not billed through insurance. New Horizon Chiropractic & Wellness does not accept insurance or Medicare, and we do not file insurance paperwork on your behalf. This is partially due to our medical approach to care. Insurance covers catastrophic and disease based issues. Our practice focuses on the entire patient, achieving optimal health with proactive programs and treatments many of which are not covered by health insurance.
Upon request we will be happy to provide a detailed receipt for services performed for you to submit to your insurance carrier for reimbursement provided your insurance offers "out of network" benefits for the services provided. For functional medicine, some lab tests we utilize do accept insurance and your insurance may be utilized for those services separately.

What is a Chiropractor?
A primary care doctor who specializes in spinal health and well-being. They focus on the prevention, diagnosis and conservative care of spine-related disorders and other painful joint issues. In addition to adjustments, chiropractors also provide soft-tissue therapies, lifestyle recommendations, fitness coaching and nutritional advice.
What does treatment consist of?
A very safe, specific, controlled force applied to a joint to restore proper function and mobility.
These changes impact tissues, the nervous system and other areas of the body. Chiropractic adjustments reduce pain, increase movement and improve performance.
Dr. Kosciusko may assess patients through clinical examination, orthopedic and neurological testing, posture analysis, laboratory testing, diagnostic imaging, and other diagnostic interventions to determine when chiropractic treatment is appropriate or when it is not appropriate.
Dr. Kosciusko will readily refer patients to the appropriate health care provider when chiropractic care is not suitable for the patient’s condition, or the condition warrants co-management.
Flexion Distraction is a treatment in which the joints of the spine are tractioned through gentle motions which decompress the spine. Specifically great for disc injuries and osteoarthritis.

How Should I prepare for my functional medicine appointment?
Please follow these steps to prepare for your appointment:
Dr. Kosciusko will spend time with a detailed exploration of your detailed personal and family history, the circumstances around your first symptoms, and the experiences you may have had to uncover the underlying causes of your health problems.
Together, a map of dysfunction of the patient's individual health will be created to further determine what specific individualized test are warranted. (Saliva testing, genetic testing, stool testing, urine testing, blood testing).
A thorough careful physical examination will be performed.
Time and effort invested up front in initial consult and patient questionnaire before the appointment. Many people find that working with Dr. Kosciusko helps them get to the bottom of perplexing problems that have eluded other clinicians for years.
For new functional medicine patient appointments, we require a credit card to be placed on file at the time of scheduling. Seven days (7) prior to the appointment, patients will receive a call to confirm their appointment. If the appointment is not confirmed at least seven days prior to the first visit, or if the initial appointment is canceled within seven days, the patient will be charged 50% of the new patient appointment fee. Once the appointment is confirmed the patient will received automated email and text reminders two days prior to the appointment. For follow-up appointments, payment is due at time of service.
